Ben,
MAM says 6 sheets of 3/4 plywood. It can be done with 5 with very
careful layout. I recommend you spring for a "premium" top (2 sheets).
We used birch plywood and have never regretted the extra cost. Use a
plane to get the top edges flat and smooth as required before you put
the top on. Use glue and drywall screws. Put in extra cross braces and
gussets where you think they will aid triangulation. At each on the 6
feet glue in a corner block with a 1/2 - 13 coupling nut so bolts can be
screwed in from the bottom for leveling. Add a shelf underneath for
additional storage. Use the MAM packing crate if you have it for the
table shelf.
Right off, buy a "Smart Level" to assist in leveling the table
construction and many continued uses during further construction. Also
suggest a 4 foot bubble level to use for a flat straight base for the
Smart Level over greater spans as well as any rough leveling.
You will be glad you took the time to construct the build table
accurately.

PS - We found a 3 shelf materials rack of 2 x 4's and the MAM crate
parts to be useful. It has 4 uprights and 3 - 2 x 4's the long way and
across the ends for shelf supports. Dimensions are 6' high x 8' long x
4' wide. Goes a long way to avoid shop clutter.
