Use of washers

Hi all,
Just curious what the 'acceptable practice' was
regarding washers under bolt heads?
My 'Standard Aicraft Handbook' states you should
always use at least one, but no more than three,
washer(s) under a nut. It doesn't mention under the
bolt head? In some cases the manual for my SR doesn't
state the use of washers. Wondered what the accepted
practice was?
